Real bridge vs toothpick bridge

What is the difference between a real bridge and a toothpick bridge?

Nothing (and everything).

The principles remain the same. There is no difference in the structural engineering used in a real bridge and the structural engineering used in a toothpick bridge. The forces are still transferred according to the way engineers understand it.
